I'll try to explain but it might be confusing: 1). I'm assuming you mean what happens when a catcher misses the ball? If there are runners(batters) on base they may advance when the catcher doesn't catch the pitch. Otherwise, nothing really happens except for the pitch being called a "ball" by the umpire.
2). The only ways to get to first base are: by the batter getting a hit, by drawing a "walk" (four balls before three strikes), by being hit (physically) by the pitch, by making it (safely) to first base on a fielders choice (this happens when a batter hits a ball into the field of play but an out is made at another base).
