I think Rof16 (or is it QF) till SF are two-legged affairs.

EDIT : From Round of 32 till SFs it is two legs.

I thought that when a player leaves the pitch he needed the referee's approval before re-entering the field. I've never seen a goal quite like Bale's.

I think this only applies when the referee sends the player out as in injuries or blood on shirt
